The resulting ketone is protected as a ketal, then subjected to benzylic bromination and Kornblum oxidation to install a major ketone functionality. Further, asymmetric transfer hydrogenation using a ruthenium catalyst introduces the first chiral alcohol, and acid treatment removes the ketal, which gives a crystallised chiral intermediate. Then, this intermediate is fluorinated with Selectfluor, followed by another asymmetric transfer hydrogenation to achieve dynamic kinetic resolution. After isolating the desired fluorinated alcohol, the final stereocenter is set through deoxyfluorination using perfluoro-1-butanesulfonyl fluoride (PBSF), which produce high purity Belzutifan as the final product.

Belzutifan is a small-molecule inhibitor of hypoxia-inducible factor-2 alpha. It appears as a white to light brown powder, which is completely insoluble in water but demonstrates good solubility in acetonitrile, dimethoxyethane, and acetone. The molecular formula of the compound is C17H12F3NO4S, and its molecular weight is 383.34 g/mol. It is sparingly soluble in ethyl acetate and very slightly soluble in isopropanol and toluene. The compound is marketed under the brand name Welireg. Belzutifan is administered orally as tablets and is indicated for the treatment of von Hippel-Lindau disease-associated tumours. The compound is a known toxicant and may cause severe anaemia and hypoxia. It has the potential to cause embryo-fetal toxicity when administered to pregnant women and can make some hormonal contraceptives ineffective. Extended exposure to this medication may result in serious health problems, including anaemia, fatigue, headache, dizziness, increased creatinine levels, and nausea. Therefore, careful monitoring of haemoglobin levels and oxygen saturation is advised during treatment.
